First Visit

As a new patient to our practice, your initial appointment will be a complete set of electronic records which consists of charting, any necessary digital radiographs and a comprehensive initial exam by the dentist. 

If your previous dentist has taken x-rays, please request that they forward them to our office or we can provide you with a form to fill out and we will request them for you prior to your first visit with us. 

If you have a history of a heart condition (heart murmur, rheumatic fever), an artificial joint implant or have taken the medication Fen-Phen or Redux, you may need to take an antibiotic before your dental appointments. Please be sure to advise us of this 24 hours prior to your appointment.

Please read our Financial Policy page and if you have dental insurance, please bring your insurance card with you. This will allow us to submit your claims for you.

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian.
